Sinclair Update

In light of the comments made by Dr. Balachandra yesterday, Dr. Brock Wright today reviewed the security tapes showing Mr. Brian Sinclair's arrival at the HSC emergency department on September 19, 2008. He also reviewed the subsequent Administrative Review undertaken by Health Sciences Centre. Dr. Wright had not reviewed the tapes previously to avoid any suggestion of interference while the incident was being reviewed.

The tapes show Mr. Sinclair being dropped off in the emergency department by the driver of a white minivan that is part of a taxi service for people with disabilities. The driver of the van wheeled Mr. Sinclair into the Emergency Department in the area of the triage desk. Mr. Sinclair then wheeled himself into the line at the triage desk where the triage nurse on duty was seeing the one person ahead of him. A triage aide then approached Mr. Sinclair, spoke with him and wrote something down on his clipboard. Mr. Sinclair is then seen wheeling himself into the waiting area. This is consistent with the findings of the internal administrative review.

As Dr. Balachandra said in his statement and as we have stated earlier, Mr. Sinclair was never seen by a triage nurse and was not triaged or registered to be seen by an emergency physician. Again, this is clearly not the fault of Mr. Sinclair but was the result of a breakdown in our system. In response, changes were made shortly after Mr. Sinclair's death to ensure that this could not happen to someone else.

The security tapes and all other relevant information we have will be entered into evidence in the Inquest that the Chief Medical Examiner called yesterday. We have, and will continue to cooperate fully with the Office of the Chief Medical Examiner. The tapes, administrative review and all other relevant information will be submitted as evidence at the Inquest.

We look forward to the Inquest process and the opportunity to learn why Mr. Sinclair was not triaged and if there are other changes to the processes in our emergency departments that should be made.
