Healthy Aging Resource Teams do community-based health promotion with adults age 55 years and older. The role of the team is to increase awareness about injury and illness prevention, and provide education and support for management of chronic diseases, including Type 2 diabetes. Healthy Aging Resource Teams consist of health care professionals such as a nurse, occupational therapist, social worker or dietitian. Teams hold health clinics, group education workshops and events at a variety of community sites including apartment blocks, malls, seniors' centres and faith based facilities. Individuals 55+ can also arrange appointments with a health care professional for individual diabetes education and follow up. Referrals to other needed services are also offered.

Diabetes Care, Education and Self-Management Support Services:
- Group Diabetes Management Classes (Diabetes Series - Separating Fact from Fiction) topics include nutrition for diabetes management, healthy eating and food skills, exercise, medications, complications of diabetes, and blood sugar testing.
- Individual Diabetes Management Counselling such as therapeutic nutrition counselling, healthy eating, exercise, medications, complications of diabetes and blood sugar testing.
- Family Supports
- Support with accessing services that are important to diabetes management such as exercise programs, foot care, smoking cessation, support groups and additional resources
