Service Overview

TeleCARE / TèlèSOINS is a telephone (or telehealth) based self-management program for those with or at high risk for developing Type 2 diabetes.  People receive education, support, personalized coaching and tools to self-manage their condition with the support of a nurse and registered dietitian.  The frequency of telephone counselling sessions is organized for each client based on their needs and preferences.  TeleCARE / TèlèSOINS services are also available for Manitobans diagnosed with heart failure

Eligibility

Adult residents of Manitoba with or at high risk for developing Type 2 diabetes.  Manitoban adults with heart failure are also eligible for support from TeleCARE / TèlèSOINS Manitoba.

Referral Process

  • Self‐referral
  • Referral from physician (MD), nurse practitioner (NP) or physician assistant (PA)
  • Referral from other health care professional or social service agency
  • Referral form

Diabetes Care, Education and Self Management Support Services:

  • Individual Diabetes Management Counselling (on topics such as health eating, exercise, smoking cessation, medications, complications of diabetes, and testing blood sugar).
  • Personalized support and coaching towards our health behavior change goals is also provided.
