A key part of social prescribing is the referral form, which service providers can use to connect an older adult directly with a Senior Resource Coordinator. Once the referral form is submitted, the coordinator will follow up with the older adult to assess their needs, listen to their concerns, and guide them toward a wide range of programs and services available in their community.
