Service Overview

CDMC (respiratory focus): Assist with management and service navigation related to chronic conditions, with a focus on education and self-management plans related to respiratory conditions, such as asthma or COPD.

CDMC (diabetes focus): Assist with management and service navigation related to chronic conditions, with a focus on education and self-management plans related to diabetes, including adjusting or starting insulin. The CDMC will also provide education related to heart health, digestive issues, or other chronic nutrition concerns.

Occupational therapist:

  • Assessment of functional performance with activities of daily living and daily tasks, cognition, functional mobility, and other skills assessment
  • Individualized recommendations for equipment/assistive devices, activity modifications, environmental/home modifications, fall prevention, and self-management skills
  • Wheelchair seating assessment and prescription
  • Resource navigation; consultation and linkages to appropriate community programs
  • Collaborative goal setting related to function, health behaviours, and client’s values.

Primary Care Social Worker: Can assist in goal setting, advocacy, and service navigation related to mental health, addictions, housing, finance, employment, immigration, and services for older adults.

Clinical Pharmacist: completes medication reviews, provides consultation and education to patients and providers, and assists patients in managing chronic conditions.

Brief Treatment Counsellor: provides short term (6-8 sessions), solution focused therapy related to anxiety, depression, increasing self-worth, coping with life changes, and navigating relationships.
