About My Health Team

Why: Primary care practices are often busy, and My Health Team helps by providing access to allied health teams to support clinics and improve both the quality of care and access to services. Clinics are involved in identifying what’s needed, with physician engagement driving decisions based on feedback from partners.

Who: My Health Team is a partnership between the Winnipeg Regional Health Authority (WRHA), Manitoba Health, and partner clinics that choose to sign on to the My Health Team agreements.

What: My Health Team is part of the WRHA Primary Care Renewal Strategy, aiming to improve and integrate health services. It also aligns with other larger strategies, including FDF (Family Doctor Finder) and Interdisciplinary Team Demonstration Initiative (ITDI).

Where: The My Health Teams are organized by paired community area across the city. Each area in Winnipeg has unique needs, though some challenges are shared across communities. The teams provide care to patients through virtual visits, in-clinic appointments, home visits, and community-based services.

How the My Health Team Clinicians Help

The patient is the expert of their own life. My Health Team Clinicians acknowledge this by engaging with the patient in every step of their health plan. Clinicians work to provide holistic care, in consideration of the social determinants of health and their impact on health planning.
