Service Overview

The Rehab Clinician (PT): assessment and treatment of musculoskeletal, neurological and general mobility issues, comprehensive falls risk assessment and intervention, and exercise prescription/guidance for individuals with complex/chronic medical issues such as diabetes, congestive heart failure and COPD. The goal with every patient is to assist them in reaching their goals and to provide the tools they need to sustain a more physically active lifestyle!

CDMC (RN, Certified tobacco educator): provides consultation re: Heart Failure, Diabetes and Smoking Cessation. She can assist you with symptom management, lifestyle changes and behavior coaching with the goal to achieve better self-management of your disease and improvement of your health/wellness. Referrals include comprehensive assessment, education and on-going support in the community.

CDMC (RT, Certified Respiratory Educator, Certified Tobacco Educator): Assist patients with managing Chronic Respiratory Disease.

  • help patients with Respiratory Health System Navigation for sleep apnea, respiratory home care, Pulmonary Rehabilitation, neuromuscular disease and tracheostomies.
  • Provide education and counselling for smoking cessation,
  • Spirometry testing,
  • Inhaler education
  • Assessments for exertional Home Oxygen.

CDMC (PT): focusing on chronic pain conditions that affect day-to-day functioning and quality of life. Provides education and support to develop self-management skills for conditions such as myofascial pain, fibromyalgia, chronic fatigue and arthritis.

Primary Care Social Worker: Support with systems navigation and information on housing, income security, mental health, and community resources.

Brief Treatment Counsellor

  • Short term therapy (6-8 sessions) to clients experiencing psychosocial distress
  • Consultative services for the management of emotional & mental health
  • Brief Solution-Focused Therapy approach with CBT
  • Life stressors/transitions – Depression – Anxiety – Grief
  • Individuals experiencing psychotic symptoms such as active hallucinations and delusions or who have severe cognitive impairment are not appropriate for Brief Treatment Services
  • Individuals must not already have access to alternative counselling services (including EAP, Shared Care, FNIHB).
