How to Access Palliative Care Services

The WRHA Palliative Care Program provides services for people who have been diagnosed with a life-limiting illness. The goal of the program is to offer comfort and support, usually in the advanced stages of illness when other treatment options have been exhausted.

To explore registering with the program, talk to your doctor or nurse practitioner who will complete a referral form and send it to the WRHA Palliative Care Program. After reviewing the form and determining that the program will meet your needs, a palliative care coordinator will contact you.
