Information for Health Care Professionals
There are two main components of the WRHA Palliative Care (PC) Program services:
Consultation services and/or registration with the program
Consultation services can be accessed for patients who have a life limiting illness and are experiencing distressing symptoms (e.g. nausea, pain, psychosocial or spiritual distress). These services are generally provided during business hours, however MD to MD consults are available 24 hours, 7 days a week.
Contact the Palliative Care MD on-call through St. Boniface paging at 204-237-2053.
Telehealth and telephone consultation support are also available to health- care professionals throughout Manitoba.
To initiate a consult in any care setting from Monday to Friday, 8:00 a.m. to 4:00 p.m., call 204-478-6372.
Registration with the WRHA Palliative Care Program
A patient is eligible to be registered on the PC program if he/she:
- Has an illness that is likely to take his/her life in the next six months;
- Is referred by a physician or nurse practitioner;
- Is no longer receiving aggressive disease-focused treatment (most often this is chemotherapy) or requiring monitoring for and treatment of potentially serious complications;
- Has chosen a comfort-focused approach including a decision to decline attempted resuscitation.
Registration with the PC Program provides access to other community-based care and services, including care coordination by a Coordinator.
