How to request to see or get a copy of your personal health information

Complete the WRHA Request to Access Personal Health Information Form (PDF, 192 KB) and submit it to the site where your personal health information is maintained. For a listing of sites and their addresses click here.

Your request should be sent to the attention of the Privacy Officer at the site where your information is kept.
