Make a Referral

Referrals to the Antenatal Home Care program can be made by physicians, midwives, and hospital staff between the hours of 8:30 a.m. and 4:30 p.m., 7 days a week.

Complete the Antenatal or Postpartum referral form and call the program at 204-792-5463 to discuss client condition, eligibility for program follow up and receive confirmation the referral is accepted.

Once accepted to program, the client can expect contact from a nurse by phone within 24 hours and an initial home visit soon after.
