TeleCARE TéléSOINS Manitoba helps you manage your Type 2 Diabetes or Heart Failure with one-to-one support, education, health monitoring, and health behaviour change coaching. Working with your primary care provider and your treatment plan, registered nurses and dietitians work with you over the phone anywhere in Manitoba to create an individualized program to support you to manage your illness and improve your health.
