WCC00017 Day Hospital Referral

  • Deer Lodge Centre (DLC)
  • Riverview Health Centre (RHC)
  • St. Boniface General Hospital (SBH)
  • Seven Oaks General Hospital (SOGH)SOGH Day Hospital Healthcare Award
  • Health Services on Elgin
  • Client address determines the Day Hospital they attend
  • Service focused on client’s age 65 years or older
  • Client referred by their Primary Health Care Provider, Community Coordinator, Hospital Care Team or Outreach Team
  • Require services of at least two of the following disciplines:
    • Dietitian
    • Geriatrician
    • Geriatric Psychiatrist (available at some locations)
    • Nurse
    • Occupational Therapist
    • Physical Therapist
    • Pharmacist
    • Social Worker
  • Services include:
    • Assessment of health and care needs
    • Treatment and rehabilitation as required
    • Transition from hospital to community
    • Health promotion and support
    • Discharge planning and community follow-up
    • Geriatrician consultation
  • Core Programs offered are:
    • Fall Prevention
      • Provides assessment and treatment for individuals for high risk of falls
      • Clients attend twice weekly for eight weeks
      • Program includes group exercise and education as well as individualized assessment/treatment
    • Community Transition
      • Focused on clients discharged from Geriatric Inpatient Units returning to Community Living
      • The goal is to provide smooth continuation of rehabilitation for those requiring general monitoring, mobility and group exercise
    • Comprehensive Geriatric Assessment
      • Provides in-depth assessment of multiple aspects of clients’ function including activities of daily living, physical well-being, memory, with focus on identified health issues
      • Often includes a home visit portion of the assessment
    • Non-specific Geriatric Rehabilitation
      • Extended assessment and general strengthening and conditioning
      • Short to medium term participation in a recreational program, OT/Rehabilitation Assistant led activities/or PT/Rehabilitation Assistant led exercise program.

Rehabilitation Day Program RDP Amputee HSC

  • Located at Health Sciences Centre (HSC)
  • Rehabilitation Day Program focusing on medical treatment, prosthetic fitting and training, activities of daily living and functional restoration with a goal of community management of Amputee patients.
  • The Rehabilitation Team members are:
    • Rehabilitation Medicine Specialist
    • Registered Nurse
    • Physical Therapist
    • Occupational Therapist
    • Social Worker
    • Rehabilitation Assistants
